Preventing Type 2 Diabetes – Small Changes, Big Impact

🗓 Type 2 Diabetes Prevention Week
📅 28th May – 1st June 2025
📍 Uplands Medical Practice, Bury
This week marks Type 2 Diabetes Prevention Week, a nationwide initiative aimed at raising awareness of how to reduce your risk of developing Type 2 Diabetes — a serious health condition that’s largely preventable.
At Uplands Medical Practice, we’re proud to support this campaign and help our patients make informed lifestyle choices that can make a real difference to long-term health.
What is Type 2 Diabetes?
Type 2 Diabetes occurs when the body becomes resistant to insulin or doesn’t produce enough. If left unmanaged, it can lead to serious complications including heart disease, vision loss, and nerve damage.
The good news? It’s preventable for many people.
Are You at Risk?
You may be at higher risk if you:
- Are aged 40 or over (25+ if you’re from a South Asian background)
- Have a family history of diabetes
- Are overweight or inactive
- Have high blood pressure
✅ You can check your risk level in just a few minutes using the NHS Diabetes Risk Tool:

How You Can Take Action
Prevention doesn’t have to mean big changes all at once.
Here are some small, realistic steps to reduce your risk:
- Move more – aim for 30 minutes of activity five days a week
- Make healthy food swaps – try wholegrains, vegetables and lean proteins
- Lose just 5% of your body weight if overweight – it can cut your risk significantly
- Cut back on sugary drinks and processed foods
